For Doug Brown, 2021 marks the end of his second term on the Board of Directors for Praxis Spinal Cord Institute. It also coincides with another honour, being named a Member of the Order of Australia in recognition of his significant service to medicine, particularly to spinal cord injury. As a physician with a forty-year career working in research, Doug sees a significant paradigm shift ahead and Praxis leading the way as a vehicle for change both in Canada and internationally.
Jerome So completes his second term on the Praxis Spinal Cord Institute Board of Directors this June. With a toolkit of community-based values, passed down by mentors over his 25-year career, Jerome has a meaningful impact on Board culture. He knows the importance of giving back and avoiding ‘bystander syndrome’ to be a role model for the next generation.

  1. Bladder function,

  2. Skin moisture/integrity and thermoregulation,

  3. Exoskeleton (standing and walking), and

  4. Shoulder, arm and hand function.


This information is critical for these companies in their development and commercialization of products and services that will directly impact those living with SCI.


At the end of the Survey, you will be asked if you would like to enter your name in one or more of the following:

About the Survey



  • This Survey is open to all individuals 18 years of age and older living with a spinal cord injury.

  • The Survey takes about 15 minutes and can be done online on any tablet, computer or mobile device.


Protecting your privacy


Safeguards are in place to ensure your privacy. Data collected in the Praxis SCI Accelerate Survey follows strict privacy practices under the BC Freedom of Information and Protection of Privacy Act. The Act governs how your information is collected, who sees it and how the information is used.


Is the information I provide on the UBC Qualtrics survey tool stored in the US?


No. Qualtrics specifically configured the UBC instance to save all data to Canadian servers. The survey information collected using this tool is stored in Toronto, Ontario and backed up in Montreal, Quebec.


Is the Praxis website secure?


Yes. The Praxis Spinal Cord Institute website, https://staging.praxisinstitute.org, embraces computer network communication best practices. Praxis’ internet communication protocol protects the integrity and confidentially of data between the user’s computer and the website.


Is my personal information at risk?


No. We take website security seriously. Visitors to our site who are reading or downloading documents will not encounter any additional security risk. The content is public, non-personalized information.


What happens after I complete the survey?


Results will be made available in early fall. If you agreed to participate in a future phase, you will be contacted directly via email.

Introduction


The Praxis Spinal Cord Institute SCI Accelerate 2021 Survey (the “Survey”) has been developed to facilitate the objectives and goals of the Praxis SCI Accelerate Program 2021. This is an 18-week acceleration pilot program for health start-up companies with solutions that address care- and cure-related treatments and overall quality of life for individuals with spinal cord injury (SCI). Located in Vancouver, Canada, the program is designed for post-prototype companies to scale their innovations quickly. We pair our expertise in SCI research, knowledge translation, direct consumer engagement and commercialization.


This Survey is a part of our direct consumer engagement. We need to learn more about your experiences in four specific areas:



  1. Bladder function,

  2. Skin moisture/integrity and thermoregulation,

  3. Exoskeleton (standing and walking), and

  4. Shoulder, arm and hand function.


We are interested in hearing from anyone 18 years of age or older who is living with SCI.


This Survey is being delivered using University of British Columbia’s secure survey tool, Qualtrics.


Procedures


The Survey is voluntary and available via the internet. If you decide to participate in the Survey, you may answer as many questions as you wish. The Survey includes questions to help us provide the most up-to-date critical information regarding how your bladder, skin moisture/integrity, body temperature control, arm and hand function as well as standing and walking affects you and your quality of life. The data from this survey will be analyzed to find out what the SCI consumer needs are in relation to medical devices. This information is critical for the Praxis SCI Accelerate Program 2021 cohort of companies in their development and commercialization of products and services that will directly impact those living with SCI.


It is estimated that the Survey will take approximately 15 minutes to complete.


Risks and Benefits


We do not anticipate any harm will be caused to you by completing the Survey. Some of the questions we ask may seem sensitive or personal. You do not have to answer a question in the Survey if you do not want to.


You may benefit from the Survey if these companies are successful in their efforts to develop and commercialize their products and services.


Privacy and Confidentiality


You can participate in the Survey anonymously (without providing any identifying information).

At the end of the Survey, you will be given an option to provide your contact information if you would like to enter your name in one or more of the following:



  1. A draw for a $100 Amazon gift card.

Your privacy and confidentiality will be respected at all times. You will not be identified in any reports, and information that discloses your identity will not be released.


The platforms used to collect Survey and registration data are fully compliant with the BC Freedom of Information and Protection of Privacy Act. The Survey data is kept secure, and is stored and backed up in Canada.


Data Access and Storage


The Survey data will be safely stored within the secure computer network of the University of British Columbia (UBC). After the Survey data is downloaded onto UBC’s secure network folders, it will be encrypted to protect the information. Access to Survey data will be strictly limited to the Survey project team. All analysis of Survey responses will be reported in aggregate to ensure participant confidentiality. Data will only be used for the purposes of the Praxis SCI Accelerate 2021 Program as outlined in this Form.


Sharing the Results


Only the generalized results from this Survey will be made available to the companies participating in the Praxis SCI Accelerate 2021 Program and may be publicly used by Praxis on their website in the form of reports and graphics. Any information released will be summarized and will not identify any participant.

Participant Consent


Your participation in this Survey is completely voluntary.


You are free to withdraw at any time. At the end of the data collection and, once the data have been analyzed and made publicly available at an aggregate level, participants will be unable to withdraw their Survey data.


This study has been reviewed by Veritas Independent Review Board (IRB). If you have any questions about your rights as a research participant or the Investigator’s responsibilities, you may contact the Manager of Veritas IRB 24 hours per day and 7 days per week at 514-337-0442 or toll-free at 1-866-384-4221. An IRB is a group of scientific and non-scientific individuals who perform the initial and ongoing ethical review of the research study with the subject’s rights and welfare in mind. If you have any study-related comments, complaints, or concerns, you should first contact the study investigator. Please call the IRB if you need to speak to a person independent from the Investigator and the research staff, and/or if the Investigator and the research staff could not be reached.
